Job summary
An exciting opportunity has arisen to join our experienced team of specialist nurses in the delivery of Cardiac Rehabilitation. We are seeking a nurse who possess a nursing background in Cardiology and who has completed a Masters in advanced clinical practice, or who is already working towards completion.
You will care for patients following cardiac events as part of the Cardiac Rehabilitation Team and the extended cardiology service providing risk factor reduction, medicine and symptom management according to local and national guidance in clinic (both telephone and face to face) and community settings. You will be a source of expertise for the patient, family and colleagues.
Main duties of the job
The post holder will join the multidisciplinary team to contribute to the delivery of cardiac rehabilitation care in a variety of settings. The role will involve implementing cardiac rehabilitation interventions in Conwy and Denbighshire and also link with Cardiac Rehabilitation colleagues across North Wales and the rest of Wales to ensure service improvement.
The successful post holder will be required to take an active role in managing the cardiac rehabilitation caseload under the care of the specialist service. The post holder will promote independence, confidence and improved quality of life for both the client and their family where necessary. You will require excellent communication and motivational skills and be able to work effectively both within a team and on your own.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
