Job summary

This is an opportunity to work in a brand-new building; The Louisa Martindale Building is opening soon. We are excited to be moving into level A8 Which is a 27 bedded Dementia Ward. With its spacious bays, side rooms, amazing views of the sea and the Downs, allowing the best environment to care for our elderly patients.

We specialise in delivering care to the Elderly group of patients with cognitive impairment, living with a diagnosis of Dementia or who may have Delirium/Acute confusion. Patients are assessed and treatment is implemented, a 'discharge plan' is formulated for this complex patient group to leave the acute hospital setting safely. Patient - centred care delivered, focusses on preventing deconditioning together with maintaining the patients' mobility and muscle mass. The area has a great Multi-disciplinary team which includes a team of volunteers who arrange regular activities for the patients, a Flow -Co-Ordinator and dedicated ward Clerk, who all work collaboratively and cohesively together in the patients' best interest. The area is serviced by a Practice Development nurse who covers all of the Care of the Elderly (COTE) areas. A Psychiatrist reviews each patient weekly, and the area also has the services of the trust Older Persons, mental health liaison nurse (OPMHLN).

Main duties of the job

The unit philosophy is to deliver care to the highest standards whilst also caring for family members. Each patient is treated as an individual and their relatives are encouraged to complete a 'This is me' passport for the patient for the team to learn as much as possible about each patient's individual needs. Families are encouraged to bring patients' personal belongings to enhance activities of daily living and mobility for patients.

Our Multi-Disciplinary Teams are amazing, drawing on their skills, knowledge, and expertiseto give the best care possible. With us all working together we aim to try and keep our patients as mobile as possible to reduce their stay in the hospital.

Job responsibilities

  • Provides effective, safe, evidence based patient care in accordance with the Nursing and Midwifery Council (NMC) Code of Conduct.
  • Assesses, plans, implements and evaluates individualised evidence based patient care.
  • Works in accordance with ward philosophy and nursing model.
  • Effectively processes admissions & transfers and plans discharges inaccordance with UHSussex policy and standards.
  • Actively participates in promotion of education for patients, their relatives and colleagues with consideration to resources available.
  • Ensures provision of all drugs as prescribed in accordance with UHSussex policy, including parenteral administration
  • Ensures safe practice with regard to moving and handling, SHARPS and adheres to trust infection prevention policies including Personal protective equipment (PPE).
  • Provides accurate documentation of patient care in accordance withUHSussex and national policies.
