Job summary

The advanced heart failure supportive care nurse will work within an already established nursing team, caring for patients with heart failure due to LVSD. They will ensure evidence based practise and individualised care planning, working within the setting of out-patient's department and patient's homes. To work in collaboration with the wider multi-disciplinary team; Cardiology consultants, Palliative care Consultants, GP's, other cardiology/palliative specialist services and community services.

The post holder will manage their own case-load of patient's, delivering specialist individualised care, working closely with heart failure patients, their families and carers. This will include improving patients health related quality of life, by improving symptom control and providing support within the community for their complex care needs. This will include maintaining documentation and communication with both primary and secondary care, collating valuable data to evidence the need for on-going continuation of the service.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

This post is fixed term until 31 March 2024 to meet the needs of the service.
