Job summary
The post holders will work closely with the multi disciplinary team in their locality. They will offer interventions for older adults in their own home. Caseloads will consist of a range people with enduring mental health difficulties, dementia's as well as shorter term interventions with people with functional illnesses. The team promotes a recovery approach.
Main duties of the job
The main duties of the job includes providing assessment and interventions to older adults who have a range of mental health difficulties in their own home, through a recovery approach.
